Grant connected on 7-of-13 from beyond-the-arc and Sharpe made 12-of-18 from the field.
Deni Avdija finished with 26 points, seven rebounds, and eight assists.
Steph Curry led Golden State (13-14) with a game-high 48 points on 12-of-19 shooting from three-point range.
Portland (10-16) trailed by nine early in the third quarter but responded with a 17-7 run to retake the lead. Golden State would hold a three-point lead heading into the fourth. Sharpe scored 16 in the third.
The Warriors pushed their lead to 10 early in the fourth on a 10-3 run but the Blazers responded with 12-straight points to take a two-point lead with 7:24 to play.
Curry would score five-straight to give the Warriors a 126-121 lead with 2:33 to play.
Portland closed the game on a 15-10 run to secure the victory. Grant with 19 in the fourth on 6-of-9 shooting.
The Trail Blazers finished with 30 assists on the night and connected on 13-of-20 from the field in the fourth quarter.
Up Next- The Trail Blazers will get three-days off before the hosting the Sacramento Kings on Thursday evening at Moda Center. The game is scheduled to tip off at 7:00 PM.
